Form 1095-B

Form 1095-B is a US information statement reporting individuals covered by specified minimum essential health coverage and the months of coverage. Insurers, plan sponsors and other eligible coverage providers use it under the reporting rules. It records coverage rather than calculating premiums, a premium tax credit or an individual's federal income-tax liability.

What it means

The statement identifies the responsible individual, coverage provider and covered people, and the responsible individual can be the primary subscriber, employee, parent or another recipient under the rules, who is not necessarily the only person covered by the policy. Monthly reporting matters because coverage can begin or end during the year, so check each covered individual's record rather than assuming every family member has the same enrolment dates.

Minimum essential coverage is a defined category, not a claim that every medical product counts, as government programs, qualifying employer coverage and certain individual arrangements can fall within it while a supplemental benefit alone may not have the same reporting status. The provider responsible for reporting depends on the structure: an insurer reports insured coverage, while a plan sponsor may report self-insured coverage.

An employer should identify which party owns the reporting duty rather than assuming the payroll vendor or insurer handles everything. Employer size and plan type can change the form used, as the IRS instructions generally direct applicable large employers with self-insured plans to report relevant enrolment in Form 1095-C, Part III, while smaller self-insured employers and other providers can use Form 1095-B under the applicable conditions.

Marketplace individual coverage is generally reported on Form 1095-A instead, which includes information used for premium tax credit reconciliation. Form 1095-B is not a substitute for the Marketplace premium and advance-credit figures merely because it confirms coverage.

A person can also receive more than one coverage statement, from different providers during the year or from overlapping plans, which does not necessarily mean either is wrong, but the months and covered individuals should be checked. The federal individual shared responsibility payment is zero for tax years beginning in 2019 and later, which does not mean coverage reporting disappeared.

It also does not decide whether a state or other jurisdiction has its own coverage or tax requirements. A coverage statement therefore should not be described as an automatic federal penalty calculation, because it supplies a record under the reporting framework and tax treatment and any separate obligations need review under the rules applicable to the person and year.

The IRS instructions allow an alternative manner of furnishing statements when specified website notice and request conditions are met, so the absence of an automatically mailed copy need not mean the provider failed to report coverage. Check the provider's notice and request process before reaching that conclusion.

A request process is not permission to make confidential records public, because the form contains identifiers and coverage details that should be handled through an appropriate private channel. Errors in names, identifiers or enrolment months can require a corrected form, so contact the coverage provider rather than changing its reported facts without coordination, and retain the corrected statement with the relevant records so the reason for any difference is clear.

For a non-finance manager, the operational task is accurate enrolment data and a clear reporting handoff. Coordinate benefits administration with the provider or plan sponsor, and do not promise employees a tax result simply because they receive a health coverage statement.

An insurer issues Form 1095-B for a family enrolled in employer-sponsored insured coverage. The family checks the covered people and months, recognising that the statement does not calculate a Marketplace premium tax credit.

A small employer operates a self-insured plan. Its benefits manager checks the reporting instructions and confirms the plan sponsor's responsibility instead of assuming an insurance carrier will issue every required statement.
