What it means
Original Medicare covers many health services, but members still face cost sharing, and a Medigap policy sits alongside Original Medicare and pays certain gaps under the chosen plan. Medicare's official guidance explains that plans are sold by private insurers, have standardised lettered benefits in most states, and charge a separate premium.
The name is easy to misuse, because Medigap does not ordinarily add routine dental, vision, hearing aids or long-term care coverage. It is also not the same thing as Medicare Advantage, which is a different route to receiving Medicare benefits.
A buyer must check eligibility and the current official comparison for the state where the policy will be issued. State rules can differ, so do not assume a plan available in one place or year is available everywhere.
Timing matters. Medicare describes a Medigap open-enrolment period tied to eligibility, during which buying rights are generally stronger, and outside that window an insurer may apply medical underwriting in many circumstances, subject to federal and state protections.
For a business owner helping a retiring employee or family member, the decision is about total cost and freedom to use providers. Compare the Medigap premium plus Original Medicare premiums and possible drug cover with the out-of-pocket costs and network rules of alternatives.
A lower monthly price is not necessarily a lower-cost year after a major illness. The process should remain the retiree's decision, using the official Medicare plan comparison and state insurance help programmes.
Verify any agent's claim against current policy terms before paying, because a private sales brochure is not the governing description of Medicare eligibility. There are practical questions at the sale counter too: ask whether premiums can change as the policyholder ages, whether the insurer offers the chosen standard plan in the state, and how claims are coordinated with Original Medicare.
Do not cancel an existing policy until replacement coverage is effective and the terms are understood.

Option A is Original Medicare with a Medigap policy and a drug plan: $2,400 of Original Medicare premiums + $1,800 of Medigap premiums + $480 of drug-plan premiums + $600 of remaining out-of-pocket costs = $5,280 in a quiet year. In a year with a major illness the remaining costs might rise to $1,200, giving $5,880. Option B is an Advantage plan with drug cover included and no extra premium: $2,400 + $0 + $0 + $1,500 = $3,900 in a quiet year, but if a major illness pushes out-of-pocket costs to $7,000 the total is $2,400 + $7,000 = $9,400.
